The Last Living Rose chords (first attempt!)

Hello,
So far this is the basic version I've worked out for The Last Living Rose on guitar.
Hopefully somebody who is more musical can use this as a template and improve upon it! It's a bit dodgy in places, but its ok for strumming along (and/or wailing along
).
The Last Living Rose
[Capo on 5th fret] Standard tuning
Chords used:
Em: 022xxx
G5: 32xxxx
C: x32010
D: xx0232
Verse: Em, G5, C, G5
Up until the ‘fog rolling down behind the mountains’ bit : C G5 C G5
At the end of the word ‘captains’ back to Em G5 C G5
Again at the ‘past the Thames River glistening like gold’ play C G5 C G5
Up to ‘hastily sold’ at which point continuously strum C
On the first ‘nothing’ continuously strum D until after she shouts second ‘Nothing!’
Straight back to Em, G5, C, G5 x 4
For the bridge:
D, C, G5
Let me watch night fall on the river
D, C, G5
The moon rise up and turn us silver
D, C, G5
The sky move, the ocean shimmer
D, C (stay on C...)
The hedge shake
The last living rose
Em, G5, C, G5 (to the end)

I haven't worked out the notes being played over the chords at the end, but I may add them if I can. Or as I said, some virtuoso can feel free to sweep in and correct this version!
